Care Coordination

Each client is assigned to a Care Coordinator who makes sure all the necessary services are in place and working smoothly. We offer that care with compassion, friendliness and a genuine concern for the well-being of our clients.

The Care Coordinator is available to answer any questions or concerns the client or service provider may have. We work closely with Community Care Access Centre, Community Care Concepts and other local agencies to refer and take referrals for client service.
